Early Identification of People at-risk of Hospitalization: Hospital Admission Risk Prediction (HARP) - a new tool for supporting providers and patients report describes a tool to identify patients at high risk of readmission to hospital within 30 days or 15 months of discharge.

The HARP model uses 5 patient characteristics to help improve the quality and efficiency of targeted care for at-risk patients. A more complex model with 10 patient characteristics is also available.

The tool and report were developed in partnership with Health Quality Ontario.
