Primary Health Care Information (PHCi)
Overview
The Primary Health Care Information program at the Canadian Institute for Health Information (CIHI) collaborates with key stakeholders across Canada to address priority primary health care (PHC) information needs.
Our goal is to establish new pan-Canadian data sources which can be used to better understand PHC across Canada, report on PHC indicators and inform health policy and decision-making at various levels.
Current Initiatives
The Primary Health Care Information (PHCi) program area at CIHI is working collaboratively with stakeholders from across Canada on the following initiatives:
PHC Indicators Electronic Medical Record Content Standards |

In March 2009, CIHI released version 1.1 of the PHC Indicators Electronic Medical Record (EMR) Content Standards to support the collection of standardized data through EMRs.
The pan-Canadian adoption of PHC EMR data content standards will result in more complete and comparable PHC data in the areas of access, quality and outcomes, including chronic disease prevention and management.
CIHI consulted with stakeholders across Canada and worked with an external expert group to develop and refine the data standards and align, where appropriate, with pan-Canadian and international standards. It is expected that version 1.1 will serve as a useful starting point for testing and updating these standards with the goal of achieving pan-Canadian use of a common set of PHC content standards.
CIHI will work with the jurisdictions and other key stakeholders to promote the use of common content standards in PHC EMRs across Canada.

PHC Voluntary Reporting System Prototype |
The PHC Voluntary Reporting System Prototype successfully demonstrated the feasibility of collecting EMR data in areas of diabetes, coronary artery disease, hypertension and depression from interested PHC providers and assessed the quality and structure of a sample of EMR data. Read More...

The PHC Voluntary Reporting System Prototype successfully demonstrated the feasibility of collecting EMR data in areas of diabetes, coronary artery disease, hypertension and depression from interested PHC providers and assessed the quality and structure of a sample of EMR data.
This prototype project will continue to develop and evolve in scope.
CIHI’s long-term goal for this project is to provide a rich and comprehensive source of PHC data that can be used to better understand the interrelationships between various aspects of PHC.
As one aspect of this project, CIHI is providing expertise and support to the Canadian Primary Care Sentinel Surveillance Network—a pan-Canadian chronic disease sentinel surveillance network being developed by primary care research networks in collaboration with the College of Family Physicians of Canada and funded by the Public Health Agency of Canada.

CIHI is in the preliminary planning stage of a pan-Canadian practice-based survey of patients and providers. If this is implemented, this type of survey would provide new PHC data required to measure PHC performance and examine the interrelationships between factors such as quality, access, continuity, coordination, cost and patient and provider experiences.

Enhance and Use Existing PHC Data Sources |

For patient and provider surveys, CIHI increased sample sizes and aligned survey questions to the CIHI PHC indicators. Enhanced survey sources include the Canadian Survey of Experiences With Primary Health Care (2008), the Canadian Community Health Survey (CCHS, 2007) diabetes care module and the National Physician Survey.
This work enhanced the availability of priority PHC indicator data at multiple levels.
Pilot work is also under way with stakeholders to explore options to expand the use of existing PHC data sources, such as patient-level fee-for-service physician billing data for use in new analyses.
Additionally, CIHI will be working with jurisdictions to explore ways to enhance the quality and completeness of PHC data for alternative payment (non-fee-for-service) activities.
These projects will further support jurisdictions’ requirements for more comprehensive, comparable data to evaluate PHC renewal efforts and inform decision-making.

PHC Analyses and Reports
CIHI will use existing and new PHC data sources to produce analytical reports that contribute to the understanding of PHC across Canada. Where possible, CIHI will also increase the availability of reliable, pan-Canadian PHC data for use by PHC providers, researchers, health system planners and policy-makers.
Primary Health Care Indicators Chartbook (December 2008) offers an illustrative example of how PHC information can be used to populate 23 PHC indicators and highlights the need for enhanced data sources to enable more robust reporting.
Experiences With Primary Health Care in Canada (summer 2009) highlights Canadians’ experiences with PHC access, health promotion, chronic disease prevention and management, and quality at the pan-Canadian and provincial levels using data from the Canadian Survey of Experiences With Primary Health Care, 2008.
Diabetes Care Gaps and Disparities in Canada (December 2009) uses data from the CCHS (2005 and 2007) diabetes care module to examine variations in diabetes care across Canada.
Experiences With Chronic Disease Prevention and Management Across Canada (fall 2009) explores PHC experiences among people with chronic conditions using data from the Canadian Survey of Experiences With Primary Health Care, 2008.
